Both. The padded panel wraps under the belly and extends back to cradle the hip area, and the spring bar along the top spreads the lift across the whole back end. This is the main reason people pick it over a basic sling that only supports the belly and tends to concentrate pressure in one narrow spot.
The three-point fit (velcro, buckle, and elastic band above the tail) is designed to hold position, but any harness can shift a little during active movement. Most people find that once the straps are dialed in for their dog and used alongside a leash on walks, it stays where it needs to be.
